Children With Special Healthcare Needs

The Children with Special Health Care Needs program is provided by Yakima Valley Memorial Hospital at Children's Village. Families of children and youth with chronic health conditions or developmental delays, living in Yakima County can receive care coordination and family support.

A Family Care Coordinator can provide families with:

• Assistance in linking to services and programs in the community such as medical services, school programs, specialized equipment, transportation, and interpretation.
• Informational support such as community resources, health insurance programs, and education materials.
• Emotional support that links familias to support groups.

Referrals can be made by families, medical providers, schools, caseworkers or anyone who has a need for information, resources and support.
